Integrative Pulmonary Clinic (IPC) offers both short-term and long-term plans.
Traditional Medicare
For those with Traditional Medicare:
First visit: 90 minutes Clinic Visit or Home Visit
Follow up visit on Telehealth or Clinic Visit
Personalized care directly with Dr. Sergew
Evaluation of general heath, including medical, social, family history
Evaluation of vitals when possible
A personalized plan of care
Orders for any necessary testing and medications
Coaching to address preventative care
Review and evaluation of medical records
Spirometry testing for simple breathing tests
Breathing exercises
Your responsibility:
Standard copays and deductibles
Important:
Services that extend beyond standard Medicare coverage, including integrative care and extended time, may involve additional fees. These will always be discussed with you in advance.
Note: Labs, full lung function testing, emergency services and x-rays/scans are not available on site. Testing can be offered offsite and will be billed to your insurance.

Fee-for-Service
Fee-for-service designed for patients seeking longer visits, flexibility, and a comprehensive approach to care. This includes:
Initial Consultation (90 minutes) for $350
Comprehensive pulmonary and integrative assessment
Follow-Up Visits: $250 per hour or $150 for 30 minutes
Telehealth or Clinic visit
Focused pulmonary care with integrative support
Home visits available for patients who prefer care in their home. Travel time billed at the same hourly rate.
Care includes:
Personalized care plan
Orders for any necessary testing and medications (which can be ordered using insurance)
Coaching including breathing exercises, meditation, dietary changes, sleep optimization and other validated integrative care practices
Coordination with primary and other specialists with clear communication of care plan
Medication review
Advanced care planning (living will, three wishes, etc.)
Review and evaluation of medical records
Spirometry testing
Sleep studies available through your insurance
Smoking cessation counseling
Note: Labs, full lung function testing, emergency services and x-rays/scans are not available on site. Testing can be offered offsite and will be billed to your insurance.
Visits are direct pay to IPC. If you do not have Traditional Medicare, we do not bill your insurance. You can use your health savings account or you can submit your bill from IPC to your insurance for reimbursement.

6 Month Membership
Membership is designed for patients who want ongoing, high-touch, coordinated care. This includes:
Price: $800 per month (minimum 6 months). This includes:
Initial Consultation (90 minutes)
Comprehensive pulmonary and integrative assessment
Follow-Up Visits (up to 4 hours per month)
Telehealth or Clinic visit
Focused pulmonary care with integrative support
Home visits available for patients who prefer care in their home.
Care includes:
Personalized care plan
Help in managing home machines such as BPAP, ventilator, cough device, suction device, etc.
Orders for any necessary testing and medications (which can be ordered using insurance)
Coaching including breathing exercises, meditation, dietary changes, sleep optimization and other validated integrative care practices
Coordination with primary and other specialists with clear communication of care plan
Medication review
Advanced care planning (living will, three wishes, etc.)
Review and evaluation of medical records
Respiratory therapy evaluation when needed
Access to Dr. Sergew via phone or secure messaging during office hours and in between visits
After-hours communication when clinically appropriate
Travel-related pulmonary planning
Spirometry testing
Sleep studies available through your insurance
Smoking cessation counseling
Same-day or expedited appointments when appropriate
Free group visits and events
