Integrative Pulmonary Clinic offers both short-term and long-term care.

Conditions We Treat

Amyotrophic lateral sclerosis (ALS) or Lou Gehrig's disease
Other causes of neuromuscular weakness such as muscular dystrophy, diaphragmatic paralysis, etc.
Other pulmonary diseases such as: chronic obstructive pulmonary disease (COPD), asthma, chronic cough, recurrent bronchitis, vocal cord dysfunction, etc.

For patients who would like two visits to address new and concerning symptoms

Two Visit Plan

$1,500

Two Visit Plan Includes:

Visits:
- First visit: 90 minutes at home or telehealth
- Follow up visit in 7–10 days: 30–60 minutes
Personalized care directly with Dr. Sergew
Evaluation of new symptoms
Evaluation of general heath, including medical, social, family history
Evaluation of vitals when possible
A personalized plan of care
Orders for any necessary testing and pharmapuedical medications
Coaching to address preventative care, such as breathing exercises, sleep, meditation, dietary changes, exercise, and other validated alternative care practices
Access to Dr. Sergew during office hours via phone or email in between visits
Review and evaluation of medical records
Spirometry testing for simple breathing tests
Breathing exercises

Note: Labs, full lung function testing, emergency services and x-rays/scans are not available on site. Testing can be offered offsite and will be billed to your insurance.

Visits are direct pay to IPC. We do not bill your insurance. You can use your health savings account or you can submit your records from IPC to your insurance for reimbursement.

For patients who would like frequent visits to address symptoms and chronic diseases

6 Month Membership Plan

$5,000

($800/month with a minimum 6-month commitment)

6 month membership plan includes:

Personalized care from Dr. Sergew
- Telehealth visits weekly or as needed
- Monthly home visits in the Denver, Colorado and within 120 miles radius
Evaluation of general heath, including medical, social, family history
Help in managing home machines such as BPAP, ventilator, cough device, suction device, etc.
Orders for any necessary testing and medications
Coaching including breathing exercises, meditation, dietary changes, and other validated alternative care practices
Coordination with other specialists with clear communication of my plan of care with all other physicians that also care for you
Medication review
Advanced care planning (living will, three wishes, etc.)
Review and evaluation of medical records
Spirometry testing for simple breathing tests
Respiratory therapy evaluation
Breathing exercises
Access to Dr. Sergew via phone, email or text during office hours
Access to Dr. Sergew email or text in between visits
